This explains what we collect when you use this website or ask us for care, why we collect it, and what you can do about it.

Green Oak Homecare Services is the trading name of Green Oak Homecare Services Inc., a company incorporated in British Columbia. We are responsible for the personal information described here.

The short version

What we collect

When you fill in the booking form

Your name, phone number, email address, the city you are in, and whatever you write in the message box.

Please read this part. The message box asks what kind of help you are looking for, and people often answer with details about a health condition, a diagnosis, or a hospital stay. That is sensitive information. Share as much or as little as you want. A first name and a phone number is enough to start, and we can talk about the rest on the phone.

When you become a client

To arrange care we will need more: the address where care is given, emergency contacts, care needs, and billing details. We collect that directly from you or from a family member acting for you, and we explain what we need as we go.

While you are filling in the form

Your browser keeps what you have typed so far, on your own device, so that going away to read this page does not wipe the form. It is not sent to us, it is wiped when you close the tab, and it is deleted as soon as the form is sent. There is more about this on our Cookies and Tracking page.

Automatically

Our web host keeps standard server logs, which include your IP address, browser type, and which pages were requested. These are used to keep the site running and secure.

Why we collect it, and your consent

Under BC's Personal Information Protection Act we only collect what a reasonable person would consider appropriate. We use your information to:

Sending the form is your consent for us to contact you about your inquiry. You can withdraw that at any time by telling us, and we will stop.

We do not send marketing emails. If that ever changes we will ask you first, as Canada's Anti-Spam Legislation requires, and every message will have a working unsubscribe link.

Who else sees it

We do not sell or rent personal information. It is shared only as follows.

Information stored outside Canada. Formspree is a United States company. Information held in the United States can be accessed by US courts and law enforcement under the laws of that country. We are telling you this because PIPA requires it. If you would rather not use the form for that reason, phone us on 604-880-1046 instead and nothing is stored outside Canada.

We may also disclose information where the law requires it, or where there is a serious and immediate risk to someone's safety.

How long we keep it

Inquiries that do not become care are kept for up to one year and then deleted. Client records are kept for as long as we provide care and then for the period our insurer and BC law require. If you want your inquiry deleted sooner, ask us and we will do it.

Keeping it safe

The site is served over an encrypted connection. Access to client information is limited to staff who need it for your care. No system is perfect, and we will not pretend otherwise, but we take this seriously and we will tell you promptly if something goes wrong that affects you.

If you are not happy with our answer

Tell us first and give us a chance to fix it. Contact our Privacy Officer at info@greenoakhomecare.ca.

If you are still not satisfied you can complain to the Office of the Information and Privacy Commissioner for British Columbia: oipc.bc.ca, or 250-387-5629.

Children

This site and our services are for adults. We do not knowingly collect information from anyone under 19.

Changes

If we change this policy we will change the date at the top. If the change is significant we will say so on the home page.
